Transfer of Medical Records

All medical records will be transferred to the care of Medicentres in Newmarket.  This transfer can take up to 6 months from the time of closure. Please contact Medicentres at (905) 853-0404 ext. 53383 for record requests. There may be a fee for release of medical records, in accordance with standard practices.
